Location Courtesy breakfast

Our stay was for five nights. Housekeeping seemed lax. Toilet seat literally fell off commode when we checked in. Reported to staff and it was replaced. How does that go unnoticed during room prep? Towel supply was inadequate as our stay unfolded. Sink and shower drain developed seriously foul odor by the last night. How does that go unnoticed by housekeeping?
